Model of care
How someone is met in their hardest moments can open the door to hope—or close it.
At NeighborHealth, each person is received with care, without judgment, and supported in a way that affirms their dignity and worth.
NeighborHealth’s Recovery Services Program provides compassionate, comprehensive care based on the belief that recovery is possible for everyone—and that each person’s path is unique. We use a teambased approach that integrates medical, behavioral, and social supports.
Care is coordinated by a multidisciplinary team including physicians, nurse practitioners, registered nurses, behavioral health clinicians, medical assistants, and recovery specialists.
Our program follows Boston Medical Center’s Nurse Care Model, which places nurses at the center of patient engagement, treatment planning, and care coordination. This strengthens communication across the team and helps patients navigate treatment more smoothly.
Core principles guiding our care include:
- Harm Reduction – Meeting people where they are, reducing risk, and supporting health and safety without judgment.
- Trauma Informed Care – Recognizing the impact of trauma and creating a safe, respectful environment.
- Strengths Based Approach – Building on each person’s resilience, skills, and personal goals.
- Integrated Services – Offering medication for addiction treatment (MAT), therapy, care coordination, and connections to essential community resources.
We are a licensed outpatient treatment program under the Massachusetts Department of Public Health’s Bureau of Substance Addiction Services (BSAS), ensuring the highest standards of quality, safety, and evidence based care.
At both our South End and East Boston locations, our commitment is simple: We don’t leave people out. We won’t leave you behind.
Recovery isn’t one size fits all. Our BSAS licensed model ensures care is personalized, supportive, and designed to empower individuals to achieve meaningful, lasting recovery—at their own pace, with our dedicated team by their side.
