The PACE model of care
Neighborhood PACE is part of a national network of Programs of All Inclusive Care for the Elderly (PACE).
The PACE model of care began from the belief and experience that, whenever possible, older adults with chronic care needs, and their families and caregivers, live better and more fulfilling lives when they are in their own homes and communities.

My relatives can’t believe the care I get. They can’t believe it. This place is amazing. I could never pay Neighborhood PACE back for what it’s done for us.
In some cases, there may be a monthly premium required based on your income or assets. All services must be provided by or authorized by the PACE interdisciplinary team (except emergency services). PACE participants may be held liable for costs for unauthorized or out of PACE program agreement services. For services to be covered, they must be in accordance with the care plan and approved by the interdisciplinary care team. Participants may be liable for unauthorized services. A more detailed description of the program and covered services is included in the Neighborhood PACE Enrollment Guide and the Neighborhood PACE Enrollment Agreement. Neighborhood PACE is a NeighborHealth program.

Theresa Bonfiglio
Theresa Bonfiglio grew up in the North End during the Great Depression. She was one of eight children. Her mother was an avid reader and worked as a bookkeeper, despite having only completed eighth grade. “We were very poor, but everyone was very poor. Those were the Depression days, so no one had anything. But we were very happy. Sometimes, the less you have, the happier you are,” Theresa advises.
Janet Brown
Janet Brown was born on the Caribbean island of Antigua in the late 1940s. Growing up was tough. “I had to wear my clothes for a whole week, and then wash them on Sunday,” Janet recalls. One thing Janet loved to do was knit. “We were always knitting,” she says fondly. “I’ve knitted my whole life.” Things were simpler then.
Tom Greenwood
Tom Greenwood was born and raised in East Boston. “I moved away a couple of times,” he explains, but something always brought him back. Over the years, Tom says, “I’ve seen a lot of changes; some good, some not so good.”
Ann Sullivan
When Ann Sullivan was growing up in the 1950s, Winthrop felt like a different world compared to what it’s like today. Ann’s childhood was idyllic and decidedly undramatic. Like a scene out of Ozzie and Harriet, Ann’s father came home from work every day to a loving wife who doted on their children.
PACE Eligibility & Cost

Am I eligible?
You may qualify for care through Neighborhood PACE for many reasons. Neighborhood PACE is available to individuals who meet all of the following qualifications:
- Are 55 years of age or older
- Live in East Boston, Chelsea, Winthrop, Revere, Everett, Malden, Medford, Melrose, Stoneham, and Boston’s North End
- Qualify for nursing facility level of care as determined by the Commonwealth of Massachusetts
- Have the ability to live safely in the community with services provided by Neighborhood PACE
Once enrolled in Neighborhood PACE, all care and services, with the exception of emergency care must be approved by the PACE health care team and provided through Neighborhood PACE. These provisions, known as “lock-in,” remain in effect while enrolled. Participants may disenroll from Neighborhood PACE at any time effective the first day of the following month.
What does PACE cost?
All PACE covered services are provided with no co-payments or out-of-pocket expenses for program participants. Neighborhood PACE pays for all medically necessary services covered under Medicare and Medicaid regardless of your original insurance, when approved by your health care team.
Some participants may have a monthly share of cost premium based on income. PACE programs are covered by Medicare and Medicaid (MassHealth) for eligible individuals.
PACE services are also available on a private pay basis. Many participants qualify for zero monthly cost share, or zero premium based on income.
I’ve been here for six years. It’s more than words can tell. The nurses and employees are all so nice. They’re very protective. They always look after me. I tell everyone to come to Neighborhood PACE.

Neighborhood PACE is supported through a PACE program agreement with the Centers for Medicare & Medicaid Services, MassHealth, and NeighborHealth. Neighborhood PACE is comprehensive; Neighborhood PACE participants must receive all necessary health care, including primary care and specialist physician services (other than emergency services), from Neighborhood PACE or from an entity authorized by Neighborhood PACE. Participants may be fully and personally liable for the costs of unauthorized or out-of-program services. You may request disenrollment from Neighborhood PACE at any time. Your effective date of disenrollment will be the first day of the month following receipt of your request. Please be aware that you cannot disenroll from Neighborhood PACE at a Social Security office. H2223-2024-07 – Pending CMS Review.
