Our Public Health AmeriCorps Program
As one of the largest Federally Qualified Health Centers in the country, NeighborHealth works with communities in Greater Boston to deliver a high-quality, culturally-relevant, and accessible healthcare experience to all. We are thrilled to join forces with the Public Health AmeriCorps Program to offer the next generation of public health leaders a hands-on service opportunity with some of the top community health practitioners in the nation.
Primary Care Connector service description
The Primary Care Connector (PCC) will have the unique opportunity to make a profound impact on the lives of underserved patients and their families by acting as an extension of the clinical team. They will actively involve our communities in their care by reaching out and providing education to promote preventative cancer screenings. Additionally, they will screen patients for Social Determinants of Health needs, connect them with suitable support services, and help navigate the patient’s barrier to access so everyone can get the care they need.
Essential functions include a combination of the following:
- Complete Social Determinant of Health screenings.
- Determine eligibility for community resources.
- Provide assistance to patient’s identified barriers (i.e., language, literacy) to complete forms such as: Disability, DTA benefits, Food assistance, transportation.
- Act as a resource for Primary Clinical Care teams for community resource information.
- Educate Primary Clinical Care team on how to locate and access community resource information on our internal database.
- Track all patient encounters in our digital medical record system and identified tracking systems.

Time commitment
The service position is full time (average of 35 hours weekly) to complete a minimum of 1700 hours over the course of 11 months.
