Notice to Patients
As part of our commitment to the privacy of your health information, we would like to inform you that, as the result of a mailing error, your name and your relationship to us as our patient may have been inappropriately disclosed.
On March 9, 2024, we learned that errors in the mailing list for our announcement of our name change from East Boston Neighborhood Health Center to NeighborHealth resulted in some patients’ full names being combined with a different patient’s address. This issue affected 1% of letters, or 526 patients.
No information beyond names and the fact that the person was or is a patient of NeighborHealth was disclosed. We have no reason to believe that this information has been used to harm any individual.
We sincerely regret that this incident occurred. We have taken this opportunity to review and strengthen our procedures to ensure this never happens again.

Medical Records
Welcome to NeighborHealth Medical Records! NeighborHealth maintains medical records electronically for all patients who receive treatment or services at our facilities. Our electronic medical record system is a secure and efficient way to have your health information, including appointment notes and test results, readily available to your provider so that they can make timely decisions about your treatment. We take care of your medical records and keep them private. If you need help with anything, you can email us at MRD@neighborhealth.com.
Requesting Copies of Medical Records
If you need medical records for an attorney or insurance company, please send your request with the right permission to MRD@neighborhealth.com.
If you are a patient and want your medical records, fill out the release of information form and email it to MRD@neighborhealth.com. The form is available in the following languages:
It may take up to 30 days to process your request. If you need the records sooner, please let us know on the form, and we will try our best to help you quickly. We do not charge patients for copies of their records.
Did You Know?
You can see your whole medical record on MyChart! You can download and print it, too. If you don’t have a MyChart account, please call Patient Services at 617-568-4600 to ask for one. MyChart also lets you message your care team, request prescription refills, and schedule or cancel appointments. You can use MyChart in both Spanish and English!
Making Changes to Medical Records
If you think something in your NeighborHealth record is wrong or missing, please fill out the Amendment Request form and email it to MRD@neighborhealth.com. Be as clear as you can about the date, the type of service (emergency department visit, office visit, etc.) and what needs to be changed. We will get back to you within 30 days.
Privacy Concerns
